The registration fee is $20 through April 26 if the entry is mailed in. You may fax the form through April 29. The registration fee increases to $25 on race day. Register early and save!

ChampionChip owners may provide their personal ChampionChip identification number and deduct $2 from their entry fee.

If you prefer to enter via mail, you may print out this entry form. Please fill out the entry form completely and legibly, and send it along with your check payable to The Brain Tumor Society to the Brain Tumor 5K, 13710 Ashby Road, Rockville Maryland, 20853-2903.

Please do not mail the registration form after April 26. Credit card entries may be faxed to (301) 871-0006 until 6 pm on Thursday, April 29.

On-site registration and packet pick-up will be available at Metro Run & Walk, 7516 Leesburg Pike, Falls Church, on Friday, April 30, from 4:00 to 7:00 pm and on Saturday, May 1 from 11:00 am to 3:00 pm.

You may also register on Sunday starting at 7:30 am at Freedom Plaza on Pennsylvania Avenue between 13th and 14th Streets, NW. Arrive early to avoid the long lines. The registration fee is $25 on race day.
